I tried the forms processing - OMR/Forms processing c# sample project with some HCFA forms. The results were not satisfactory.
1. I setup the anchor and two zones (Sex - Male/Female check boxes) on the template image.
2. I loaded some filled HCFA forms and clicked on "OMR Images" button.
I have attached the screenshots of the anchor and zones on the template image and the anchor and zones on the processed image.
After reading through the posts, I understand that the anchor setup on the template is not very good (not 50% black).
Though it looks like the anchor has been successfully located on the processed images, the zones drawn for male/female check boxes are shifted to the right making the recognition incorrect.
Why is this happening and what needs to be done to fix this?

Hi,
It is impossible to tell what the problem is without having the original images.
Also from the look of things, you are using really low dpi images with bad quality sampling, that would not help in any kind of processing, be it preprocessing or recognition.
